3D Printed Assistive Devices FOrm

Complete the form below to order a 3D printed assistive tool. Items are made to order in Ela Public Library’s Forge makerspace and can take up to 2-3 weeks to be ready. When your 3D print is ready, you will receive an email and can pick it up at Circulation. 3D prints cost 10 cents per gram and can only be paid for with cash.

  • Only one 3D print request per library card at a time. Once you pick up your request you may order another one.
